Patient Information

We collect medical history, ongoing symptoms, and current medication information on the first visit.

These documents and any other information regarding a patient’s health are compiled into a comprehensive health file. This allows us to understand more about the patient’s past and current health state, and we will continue adding to this file as the patient undergoes treatment. We can also communicate and work with the patient’s other health providers to provide patients with the highest chance of success. We begin with an initial consultation to get to know you and understand your health state. This way, patients will have a good understanding of what their treatment plan entails.
